Clinical assessment

Before the surgery, the severity and stability of the patient’s IHD, the presence of significant valvular disease and the status of left ventricular function should be properly evaluated.any comorbid risk factors for IHD should be documented and, in particular , the state of coexisting disease assessed.attention is paid to the presence of carotid artery diseases ,peripheral vascular disease, respiratory status, presence of diabetic complications ,significant renal dysfunction or coagulopathy . All medications taken by the patients are noted.

Risk Assessment

Risk assessment can be calculated using various scoring systems developed for cardiac surgery, including the Euro SCORE and PARSONNET .The common scoring systems stratify risk factors that are recognised which are associated with poor outcome after surgery. An rough guess of operative mortality  can be calculated and allows informed consent by the patient.

Postoperative Recovery

The majority of patients extubated a few hours after returning  from the surgery and remain in the ICU for 24 hrs or so. In some centres fast tracking appropriate patients allows earlier transfer to the recovery area or high dependency unit HDU. discharge is routinely 5-8 days after surgery.
